Saint Mary's Diabetes Wellness Program is an educational program designed to provide people with diabetes and their families with the knowledge and skills they need to successfully manage their disease. Long-term optimum health is the primary goal. Together with the patient and physician, our staff works to provide not only quality education, but the encouragement and support needed to achieve this positive outcome. We've assembled a highly specialized staff, including Registered Nurses and Dietitians, most of whom have achieved national certification in diabetes education. Their skills and expertise ensure that we deliver only high quality, comprehensive care.

Our curriculum has been designed in accordance with the National Standards for Diabetes Self-Management Education Programs as determined by the American Diabetes Association. In keeping with Saint Mary's tradition of quality care, we are proud to boast that the Diabetes Wellness Program has carried this recognition since 1997.

Services provided
Inpatient Education
The DWP staff conducts clinical rounds and assesses all inpatients referrals for appropriateness and patient needs. The staff then proceeds to work with the floor nurses to coordinate and implement a plan of care.

Outpatient Education Program
We offer a group comprehensive program for the patient who is newly diagnosed or just needs a refresher. Topics presented include an overview of diabetes, acute and chronic complications, home blood glucose monitoring, medications, hyper/hypoglycemia, nutrition, stress management and exercise. In addition, one-on-one consultations with the Dietitian, the Nurse Clinician, the Counselor and/or the Exercise Physiologist are also available for patients who may benefit from a more individualized approach.

Gestational Diabetes Program
Our Gestational Diabetes Program is designed to help expectant mothers diagnosed with gestational diabetes learn how to eat properly and monitor their blood sugars. In addition, these patients are instructed to call in to our Sugar Line once a week so that we may better assist the physician in tracking their results. Our Nurse Clinician will remain in close contact with the physician and will initiate any follow up or further instruction with the patient should it be required.

Outpatient Insulin Pump Program
This highly effective therapy may significantly reduce the onset and progression of complication associated with diabetes. After a thorough assessment to determine if the patient is an appropriate candidate for the insulin pump, our Nurse Clinicians and Dietitians coordinate a schedule with the patient to provide intensive "pre-pump" training. This is followed by an intensive two-day insulin pump "start up" that is coordinated with the team including the physician. The success rate of this program has been outstanding.

Continuous Blood Glucose Monitoring
The DWP Nurse Clinicians are certified to provide this specialized service utilizing the continuous blood glucose monitor. Through a special infusion set attached to the patient, this small device monitors a patient's blood sugars every 5 seconds for a 72-hour, on-going period. This information is then printed out and forwarded to the physician to assist in his investigation into a patient's erratic blood sugars.

Support Groups
Saint Mary's Diabetes Wellness Program has established free support groups to help meet the needs of patients who are either newly diagnosed with diabetes or those who have experienced living with diabetes for many years. These groups are open to the community for patients, friends and family members. They are scheduled once a month on an on-going basis.

Outpatient Nutrition Services
Nutrition plays a key role in managing many health conditions. As a health care provider, you have the ability to assist your patients in benefiting from services of a Registered Dietitian. Registered Dietitians at St. Mary's Center for Health Outpatient Nutrition Services will design an individual program for your patients to incorporate evidence-based nutrition research that allows the patient to eat what they like while getting the results you want in managing their health.
